Cancellation/Missed/No Show Policy We value your time and understand that there may be special unavoidable circumstances that may cause you to cancel or reschedule an appointment. In such case, please notify us no less than 48 hours (2 business days) if you enroll a health checkup and 72 hours (3 business days) for a scheduled stomach endoscopy and/or colonoscopy. You can reach us at (949) 387-2668 and/or [email protected] This courtesy allows us to rearrange medical provider’s time and/or give your reserved time to another customer who is in need of our care.
A “Missed/No Show/Late Cancellation” is defined as missing a scheduled health checkup appointment without canceling 48-hours (2 business days) prior to your scheduled time and will be subject to a fifty dollar ($50) fee. Please note that scheduled stomach endoscopy and/or colonoscopy that are cancelled with less than 72 hours (3 business days) prior for a non-medical issue will be subject to a two hundred and fifty dollar ($250.00) cancellation fee.
Payment Policy You will need to pay upfront at the time of your appointment and prior to services being provided. We accept cash, checks, and major credit cards as forms of payment for your convenience.
Returned Check Fee If your check is returned to Chiron Total, a thirty dollar ($35) returned check fee will be applied to you.
Insurance Reimbursement and Non-Covered Service Policy As part of our service, we will send a bill to your insurance or you will need to submit your claim to your insurance for reimbursement. We accept most PPO and Medicare insurance plans. You may be eligible to receive insurance coverage based on your insurance policy. However, we cannot guarantee insurance coverage. If your insurance determines that you are ineligible, and/or services we provide are excluded from coverage or not medically necessary, you will not receive any reimbursement from your insurance. Any deductible, co-insurance or non-covered services are solely your responsibility.
